Providers & facilities

  • Primary care physician name and location
  • All specialists you see regularly
  • Preferred hospital or health system
  • Any planned surgeries or new diagnoses this year

Prescriptions

  • Complete medication list with dosages
  • Preferred pharmacy (retail or mail-order)
  • Any expensive brand-name drugs or specialty medications
  • Recent pharmacy receipts if formulary tier is unclear

Coverage & enrollment documents

  • Annual Notice of Change (ANOC) if reviewing during AEP
  • Employer coverage details if still working past 65
  • TRICARE or VA benefit information for military retirees
  • List of priorities: travel, dental, predictable costs, etc.
